Choice Enrollment Forms

Please fill out the following forms to complete your enrollment for CHOICE. Some of the information will be redundant, but it is important that you fill it out for each person even if it is the same. This will help cut down on the amount of data entry we have to do on our end, so we appreciate it greatly!


Once you complete the information forms you will be directed to a thank you page, if you have multiple children please click the link to fill out an additional form for each child. Thank you!

Parent/Guardian #1 Information
At least one parent/guardian needs to receive and read the onboarding emails and materials. Please specify which parent/guardian would like to receive the onboarding emails and materials - or both! All documents will be shared in a google drive folder.
As specified in the parent contract, at least one parent/guardian needs to read and respond to the weekly emails. Both parents/guardians may receive the weekly emails. Please specify which parent/guardian wants to receive the weekly emails - or both!
While most communication is through the weekly emails occasionally there is text communication. At least one parent/guardian needs to receive and respond to occasional texts. Both parent/guardians may receive the texts. Please specify which parent/guardian wants to receive texts - or both!
What information are you comfortable sharing with other families in our school roster?
It is fun for families to have faces to go with the names they will hear their children talking about. Are you comfortable sharing a family photo? If so please attach one below.
